Independent Living Services: A Journey to Independence
Imagine waiting 13 months for a chance to reclaim your independence—feeling trapped in a nursing home while dreaming of a home of your own. This was the reality for one of our consumers until our Home Sweet Home program stepped in with determination and heart.
Our dedicated team member, Cristina, worked tirelessly to help her navigate the long, challenging process of transitioning from a nursing home to independent living. On January 6th, during a routine meeting, she received a call about an available apartment—the very one we had applied for back in October. Despite facing a stressful setback with an eviction notice that same day, hope was far from lost.
Just a few days later, on January 14th, our consumer received the life-changing news: she had secured the apartment! Then, with the invaluable support of our colleague Giselle Lanausse—who coordinated the Money Follows the Person (MFP) program and ensured her new home was fully furnished—she moved into her accessible, welcoming home by January 31st.
This remarkable transformation is a testament to the dedication of our team and the power of community support. It’s stories like these that remind us why we do this work every day.
